Step 1:

In order to download the registration form, right click on the file that you want and select "Save target As".

Step 2:

After finish filling up the registration form, please

e-mail the registration form to lariangemilang@yahoo.com.my with the subject named "registration form”.

or

fax it to 0379563219 ; 0379603237. Please write “CHAN LEE SIANG, LARIAN GEMILANG RAKAN MUDA” on the top of the registration form.

or

Post the registration form to our address.

Step 3:

Payment of RM15.00 can be made by :

Money order/ Bank draft

Buy a money order/ bank draft of RM15.00 .

Beneficiary's Bank No. : 1440-0098870-52-2 4

Beneficiary's Account Name : Chan Lee Siang and Ooi Ai Suang.

Please send an notification e-mail to lariangemilang@yahoo.com.my after you have done your payment through money order/bank draft.

or

Postal Order

Buy a postal order of RM15.00

Post it to the following address.

On the Pay to section, write it to Bendahari Universiti Malaya .

or

Transfer RM15.00 from your bank account to our bank account. Our bank bank account information is stated above.

Please send a copy of your payment as a proof that you have done your payment through .

Or

• Pay on the day you take the t-shirt. You'll be notified by e-mail when and the location to take the t-shirt.

Step 4:

After we recieve your registration form and payment, an e-mail will be send to you to notify you that we have recieve your payment and registration form. In the e-mail, we will also send to you your registration number. Please remember the registration number as you will need to use it to participate in the competition and to take your free t-shirt.
